Each pick offers three perks from the skill's pool, drawn by rarity: a rare perk has weight 3 (common 10, uncommon 6, rare 3, epic 1.5, legendary 0.5, mythic 0.2). You can take it up to 2 times; each pick stacks. See Perks.
